ABSTRACT

OBJECTIVE@#To investigate the effect of other gene mutations outside the fusion gene on the first complete remission (CR) induced by one course of induction chemotherapy in patients with core binding factor-associated acute myeloid leukemia (CBF-AML).@*METHODS@#DNA was extracted from bone marrow or peripheral blood samples of newly diagnosed CBF-AML patients admitted to the Hematology Department of the Second Hospital of Shanxi Medical University from January 2015 to January 2019. Next-generation sequencing was used for detection of 34 kinds of hematologic malignancy-related gene mutations in patients with CBF-AML, the effect of related gene mutations on the first complete remission (CR) rate in one course of induction chemotherapy was analyzed by combineation with clinical characteristics.@*RESULTS@#34 kinds of genes in bone marrow or peripheral blood of 43 patients were detected by high throughput sequencing and the gene mutations were detected in 16 out of 34 genes. The mutation rate of KIT gene was the highest (48.8%), followed by NRAS (16.3%), ASXL1 (16.3%), TET2 (11.6%), CSF3R (9.3%), FLT3 (9.3%), KRAS (7.0%). The detection rates of mutations in different functional genes were as follows: genes related with signal transduction pathway (KIT, FLT3, CSF3R, KRAS, NRAS, JAK2, CALR, SH2B3, CBL) had the highest mutation frequency (72.1% (31/43); epigenetic modification gene mutation frequency was 30.2% (13/43), including ASXL1, TET2, BCOR); transcriptional regulation gene mutation frequency was 7.0% (3/43), including ETV6, RUNX1, GATA2). Splicing factor related gene mutation frequency was 2.3% (1/43), including ZRSR2). The CR rate was 74.4% after one course of induction chemotherapy. At first diagnosis, patients with low expression of WT1 (the median value of WT1 was 788.9) were more likely to get CR (P=0.032) and the RFS of patients who got CR after one course of induction chemotherapy was significantly longer than that of patients without CR [7.6 (2.2-44.1) versus 5.8 (1-19.4), (P=0.048)]. The rate of CR in the signal transduction pathway gene mutation group was significantly lower than that in non-mutation group (64.5% vs 100%) (P=0.045), while the level of serum hydroxybutyrate dehydrogenase (HBDH) was significantly higher than that in non-mutation group [(418 (154-2702) vs 246 (110-1068)] (P=0.032). There was no difference in CD56 expression between the two groups (P=0.053), which was limited to the difference between (≥20%) expression and non-expression. (P=0.048).@*CONCLUSION@#CBF-AML patients with signal transduction pathway gene mutation are often accompanied by high HBDH level and CD56 expression, moreover, the remission rate induced by one course of treatment is low.

ABSTRACT

OBJECTIVE@#To improve the method for detecting the neutrophil CD64 (nCD64) index and to enhance the detection rate and accuracy of nCD64 index in patients with hematologic malignancies.@*METHODS@#The nCD64 index in peripheral blood of patients with hematologic malignancies combined with suspicious bacterial infection (255 cases-time) was detected by using array method. When the detection of nCD64 index in samples was interfered with abnormal cells in detection process of enrolled patients, the antibodies CD45, CD15 and CD10 were added into samples on the basis of routine detection by using the primary detection kit, in order to more accurately distinguish the neutrphils and obtain the nCD64 index. The nCD64 index as well as the efficiency of nCD64 index, PCT and CRP for diagnosis of sepsis before and after the improvement of deteation method were compared.@*RESULTS@#The samples of 60 cases were interfered with abnormal cells in detection process, out of which the samples of 18 cases failed in detection, but these samples of 18 cases all got the effective results of detection after the detection method was improved. The detection showed that the nCD64 index before and after the improvement as well as the PCT and CRP levels in sepsis group were higher than those in non-sepsis group(P<0.05). After improvement of method, the efficiency of nCD64 index for diagnosis of sepsis was suprior to the PCT and CRP, the nCD64 index obtained after the improvement of method possessed the diagnosis efficiency same as the efficiency obtained before improvement of method, moreover the detection results were more reliable.@*CONCLUSION@#For the samples of patients with hematologic malignancies interfered with abnormal cells in the process of detecting the nCD64 index, the corresponding antibodies are added into detected samples according to the kinds of hemotologic diseases of patients, in order to more accurately gate the neutrophils in paripheral blood of patients, there by the detection rate and accuracy for detecting the nCD64 index are enhanced by accurately distinguishing the neutrophils.

ABSTRACT

Objective To explore the expression profile of lncRNAs in the testis tissue of CRYBB2 gene knockout (KO) mice and its possible role in the testis development. Methods Testis tissues(n=3)from wild-type (WT) and CRYBB2 KO mice were subjected to lncRNA and mRNA microarray profiling. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were performed to group the differentially expressed lncRNAs into regulated gene pathways and functions. The correlation matrix method was used to establish a network of lncRNA and mRNA co-expression. Quantitative (q)RT-PCR was used to verify expression of some differentially expressed lncRNAs and mRNAs. Results There were 140 differentially expressed lncRNAs and 477 differentially expressed mRNAs between testis tissues from WT and KO mice. There were 12 differentially expressed lncRNAs through the analyses of the GO, with 7 up-regulated and 5 down-regulated. The KEGG analysis showed that these differentially expressed mRNAs played important roles in Ca2+ signaling, ligand and receptor interactions, and so on. The correlation matrix method established an lncRNA and mRNA co-expression network, consisting of 9 lncRNAs and 8 mRNAs, with 17 nodes and 12 connections. Furthermore, expression of gene Rsl1 was regulated by three lncRNAs, expression of gene Lpo and gene Mpo was regulated by two lncRNAs, and expression of gene Hdac1 and gene Ephb4 was regulated by one lncRNA. qRT-PCR confirmed the significant down-regulation of lncRNA A-30-P01019163 expression, which significantly down-regulated its downstream gene P2rx7 in testis tissues of CRYBB2 KO mice(P<0.05). Conclusion LncRNA is closely related to the non-crystalline lens function of CRYBB2. LncRNA A-30-P01019163 may affect testicular cell cycle and signaling pathway by regulating P2rx7 expression in the testis tissues.

ABSTRACT

With the increasing perfection and improvement of microsurgery technique in modern ophthalmology, especially the popularization of micro-incision cataract extraction and the rise of femtosecond-laser-assisted cataract extraction, the cataract extraction and intraocular lens implantation is gradually transformed from vision rehabilitation surgery into refractive surgery and people also pursue better postoperative visual experiences. However, the preoperative and postoperative corneal astigmatism is always the main factor affecting the postoperative visual effect. ln recent years, the development of design and materials of the intraocular lens ( lOL) provided necessary basic conditions for clinical application of the Toric lOL. The Toric lOL is primarily characterized by effective correction of cataract patients′postoperative corneal astigmatism, which results in the enhancement of patients′postoperative visual function. ln this paper, the design, classification and clinical application of Toric lOL were analyzed, and current problems and prospect were discussed.

ABSTRACT

<p><b>OBJECTIVES</b>To investigate if hyperinsulinemia or insulin resistance could predict the elevation of blood pressure in non-diabetic adults.</p><p><b>METHODS</b>One hundred and seventy non-diabetic adults (NGT 107, IGT 63) were included based on the screen by OGTT in 1986. Height, weight, blood pressure were measured. Plasma glucose and insulin concentration at 0.60 and 120 min during OGTT were determined at baseline. All the subjects were followed for six years with blood pressure and plasma glucose examined at the end of the study. Subjects worsening to diabetes were excluded. Insulin area under-curve (INSAUC) and insulin sensitivity index [IAI = (1/FINS x FPG)] were calculated. Stepwise regression analysis was performed to evaluate the effects of INSAUC and insulin sensitivity to the elevation of blood pressure.</p><p><b>RESULTS</b>Both SBP and DBP levels at the end of the study were increased with increased INSAUC baseline. The SBP were (119.5 +/- 2.3), (122.1 +/- 2.5), (129.4 +/- 2.4) and (128.3 +/- 2.6) mmHg, and the DBP were (78.6 +/- 1.6), (79.7 +/- 1.7), (85.2 +/- 1.4) and (84.0 +/- 1.0) mmHg from the lowest to the highest quartiles of INSAUC respectively. Pearson correlation analysis showed Age, SBP, DBP, BMI, FINS, INS1h, INSAUC at baseline were positively correlated to blood pressure levels at the end of the study. After the adjustment of Age, sex, BMI, smoking, PG2 h and blood pressure at baseline, the INSAUC was significantly correlated to blood pressure six years later, while the insulin sensitivity index was not.</p><p><b>CONCLUSION</b>The compensated hyperinsulinemia based on selective insulin resistance rather than insulin resistance to glucose per se could predict the elevation of blood pressure in nondiabetic adults.</p>
